Community Case Management
Ubuntu Care’s Adult Case Management Services are designed to provide support to adults with special needs and their families. Case managers help link individuals to resources and services in their own communities. Case managers work with adult consumers to identify their interests, strengths and vision for their desired lives, and to develop plan to help them achieve their goals.
Eligibility & Referrals
To be eligible for this service an adult must:

Have a diagnosis of an intellectual or developmental disability

Have MaineCare coverage
To confirm eligibility, contact the Department of Health and Human Services (DHHS), Office of Aging and Disability Services, at (207) 822-0358. DHHS will provide referrals to agencies in the community that offer case management services.
